The iHealth Unified Care platform is designed to empower patients with chronic diseases such as diabetes, hypertension, and obesity to take control of their health. Through this platform, patients can receive real-time feedback from their primary care physician and other members of their professional care team.

The iHealth Unified Care mobile app offers a range of features to support patients in managing their health. This includes the ability to create a customized care plan and engage in real-time chats with care team members. The app also integrates with the Health app, allowing users to track their steps and connect with bluetooth-enabled devices such as the iHealth Align blood glucose monitor and iHealth BP3L blood pressure monitor for self-tracking and sharing data in real time.

Main features of the iHealth Unified Care platform include:

• Real-time monitoring and communication with a professional care team
• Vitals measurements using iHealth Align portable glucometer, iHealth BP3L blood pressure monitor, and other bluetooth-connected devices
• Viewing of vitals data history and trends
• Collaborative meal tracking with registered dietitians through food diaries
• Care team review and feedback on food diary uploads
• Access to health information such as appointments, care team members, vitals measurement thresholds and schedules, medications, and lab test results
• Integration with Apple Health App for daily step tracking
